Important Information from the United States District Court for the District of Nebraska
The United States District Court for the District of Nebraska wishes to inform you of several important matters.
1. Revised Corporate Disclosure Forms.
The court has revised the corporate disclosure form required pursuant to Federal Rule of Civil Procedure 7.1 and Federal Rule of Criminal Procedure 12.4. In addition to the information requested on the prior form, the form now also asks a party that is a limited liability entity or an unincorporated association to list its members and their states of residence. This additional information will assist the court not only in checking for conflicts but also in making diversity determinations. The revised forms are located on the court's Web page at http://www.ned.uscourts.gov/forms/ --> "Corporate Disclosure Statement." Please note that there are separate forms for civil and criminal cases.
2. Creating an Appendix
CM/ECF Version 3.2 contains an enhancement that will allow users to compile PDF documents into a single PDF file. By selecting the new "Create Appendix" option on the docket sheet selection screen, users can select documents from the docket sheet to download as a single PDF file. Users can also automatically include either a full or abridged docket sheet. PACER fees are charged for each document selected and for the docket sheet from which the documents were selected. At present, CM/ECF cannot automatically send the Court of Appeals a Record on Appeal or an Appendix.
